To download the Survey from the KWA Download Page:
1) You need a Memory/Flash
Stick and have it correctly plugged in;
2) Single left click the "Survey 32-FStick" link and when prompted,
save it to your Memory Stick;
You do not need to be connected
to the Internet anymore - if you like, you can close your Browser.
AT THIS POINT THE SURVEY
IS READY AND CAN BE COMPLETED NOW OR LATER
To complete the survey:
1) Double click the "OPQ32FStick.exe"
file where you saved it on your Memory Stick;
2) Firstly, ensure the "OPQ32i" circle is filled (not 32n);
3) Next, fill in Name, Title (Dr/Mr/s,Ms...) and Gender;
4) Although preferred, you may leave the Date of Birth field blank - there is
no need to fill in the other information.
~ Please be honest with
yourself - accurate feedback for your benefit is reliant on this;
~ Please try to finish this questionnaire in one sitting - to 'stop and start'
will most likely undermine the reliability of the procedure and inevitably will
result in feedback inaccuracies;
~ Note that if the program is forced to exit before completion, the responses
will be lost;
~ On completion of the questions a small file should be saved to the Memory
Stick in a sub-directory called "Shlcdt".
Now you'll need to e-mail a data file as follows:
Please do the following:
1) In the sub-directory called "Shlcdt" on your Memory Stick, please
find the CDT file (it starts with the letters "Opq"...);
2) Send it to this address - service@kendallwant.com.au;
3) Later, a message will be sent back confirming receipt of the file and a reminder
for file security to:
DELETE ALL RELATED FILES FROM THE MEMORY STICK. Please ensure this is done in
any event.
