To download the Survey from the KWA Download Page:

1) Before downloading from the KWA site, create a new directory under your C: drive called "Shlcdt" ;
To do this: a) Open Windows Explorer; b) Left click C: drive; c) Under the File menu heading select New and call it "Shlcdt" ;
Once this directory has been created, return to the download page and:
2) Single left click the "4) Master 32" link and when prompted, click the Save button;
3) In the "Save as" box, and where it reads "Save in:", open the C:\ drive file you created called "Shlcdt" and click Save;
4) You don't need to be connected to the Internet anymore - if you like you can close your Browser and disconnect.
 
 

To complete the survey:

1) Explore to the new "Shlcdt" folder and double click the "Master 32" file you saved there;
2) Firstly, ensure the "OPQ32i" circle is filled (not OPQ32n);
3) Next, fill in Name, Title (Dr/Mr/s,Ms...) and Gender;
4) Although preferred, you may leave the Date of Birth field blank - there is no need to fill in the other information;
5) Read the on-screen instructions and keep the following in mind:

~ Please be honest with yourself - accurate feedback for your benefit is reliant on this;
~ Please try to finish this questionnaire in one sitting - to 'stop and start' will most likely undermine the reliability of the procedure and inevitably result in feedback inaccuracies;
~ Note that if the program is forced to exit before completion, the responses will be lost;
~ On completion of the questions, a small file will be saved to your created directory: C:\Shlcdt
 

Now you'll need to e-mail a data file as follows:

Once the information has been saved, please do the following:
1) Explore to the single file that was saved in C:\Shlcdt - it starts with the letters "Opq"...;
2) Send it to this address - service@kendallwant.com.au;
3) Later, a message will be sent back confirming receipt of the file;
4) At this point, you can delete the entire "Shlcdt" directory that was created for this process - it is no longer required.
